What happens in this design is that there are still 2 groups, however, there is not a control group that is left "untreated". There are pretests administered to both groups. One group being the testers and the other group being the control group. A post-test is administered to both after phase 1 is complete. The groups then switch and the testers now become the control group. Both groups get the treatments/drugs, etc and are post-tested again.

To sum it up, both groups eventually get the treatments and are measured at different times, which can provide, perhaps, more solid evidence that it works regardless of the time administered.)
